We are looking forward to seeing you at your appointment. It is important to us to provide you with the best service possible. Our Audiologists and Rehabilitation clinicians do their very best to keep our appointments running on time. As such, we ask that you arrive 15 minutes prior to your appointment time. If you are late, you may need to reschedule your appointment.
If you have NOT completed NEW PATIENT PAPERWORK, please be prepared to complete it prior to your appointment. We are happy to provide you with this paperwork at the Center, we just ask that you arrive in enough time to finish it before your appointment time so we can complete your registration process. It may take up to 30 minutes to complete paperwork for a child.

Prior to your appointment, you will need to download, print and complete the attached paperwork or arrive 30 minutes ahead of your appointment so as to be able to complete the paperwork here. Patients under 18 years old should choose CHILDREN’S PAPERWORK FOR AUDIOLOGY.
